Chronic Kidney Disease (CKD)

Overview

RPM 214 - Learn how to use Remote Patient Monitoring (RPM) to monitor clients who have Chronic Kidney Disease (CKD).  Providing education about the risk factors and the complications of advanced CKD is key to helping patients deal with this serious chronic illness.  You will learn about the disease management program created to monitor the risks associated with the causes of the patients’ CKD.

Learning Objective: Trainees will be able to work with patients who have Chronic Kidney Disease and use RPM for monitoring.  At the end of the badge, trainees will be able to do the following:  

  1. Define Chronic Kidney Disease  
  2. Explain the basic diagnosis/symptoms of Chronic Kidney Disease
  3. Explain the risk assessment of Chronic Kidney Disease
  4. Be able to monitor a patient with Chronic Kidney Disease
  5. Be able to educate a patient about their Chronic Kidney Disease  
  6. Evaluate a health chart of a patient with Chronic Kidney Disease
  7. Be able to enter data into RPM for monitoring a patient with Chronic Kidney Disease (with DMP)
  8. Evaluate DMP data of a patient with Chronic Kidney Disease (at critical junctions)
